Housing Stabilization Services is a recently introduced benefit under Minnesota Medical Assistance, aimed at assisting individuals with disabilities, including those with mental illness and substance use disorders, as well as elderly people, in securing and maintaining stable housing.
Transitional services provide support in creating a housing transition plan, guiding through benefit applications, conducting housing assessments, locating resources for home goods, covering essential purchases and expenses, and facilitating the move from an eligible setting to a personal or integrated community home.
Ongoing support for those with disabilities, mental health issues, substance use disorders, or the elderly, maintain stable housing. This includes managing lease obligations, relationship with landlords, crisis intervention, skill-building for independent living, and regular support.
